Owner Waiver: I hereby certify the to the best of my knowledge my dog is in good health and is carrying no infectious diseases. My dog has been current on annual vaccinations or shows immunization through my veterinarian. I currently have my dog under veterinary care and have taken steps to immunize against rabies, distemper, parvovirus and kennel cough. My dog has also been approved for soundness through my veterinarian and is in good health. I will be held fully responsible for all actions and behaviours of both myself and my dogs during classes held by Angela Lee. I understand that if my dog displays aggressive behaviour, I and my dog may be asked to leave. I will absolve Angela Lee and its members and training facility (if applicable) for any damage which may occur to myself, my dog, my vehicle and all those whom I bring to classes. All parties (owners and class instructors) will not be responsible for any liability from the actions of dogs and handlers and/or other persons attending these classes and on the premises. I understand I will be placing myself and my dog into situations which may incur injury. At no point will Angela Lee or Well Dressed Wolf Dog Training be held responsible for any injury to myself, my dog or my visitors before, during and after classes held by Angela Lee. In addition, I absolve Angela Lee and Well Dressed Wolf Dog Training from any and all responsibility if I contract COVID-19 or any other virus while participating in classes at the training facility. I understand that there are no refunds provided.
